    +1 for : Additionally, allow basic formatting of the text in the description, i.e. bold, italics, underlining.

    Would also like font sizes, indented paragraphs, numbered lists, bullet lists, & font colour.

    I need a broader choice of scale to assist with applying weighting to different questions. e.g. assessing a food product... question one: rate the flavour from 0 to 25, question two: rate the texture from 0 to 15, question three: rate the presentation from 0 to 10. Total score for the assessed food product is combined from the three aspects to give overall score out of 50 (25+15+10). As Forms presently allows, score can be rated out of max 10; whilst you could apply weighting with a multiplication factor, this would a) be an extra manual step to process the responses, and b) come at the expense of granularity. Also a zero rating is not presently possible for a mandatory question; the Form designer should be given the option of allowing responses of zero.
