For a user who creates forms and assessments at a company with more than 60,000 employees - the inability to create folders to organize your forms is curse! Imagine organizing and searching for forms (which you have to archive or keep for at least 2 years for records purposes):
    1. Assessments for new joiners (which happens every month)
    2. Shift preference surveys
    3. Assessments for in-house training

    and many many more.

    Dear Forms customers,

Thanks for your patience for the long wait on file upload.

File upload v1 – for internal survey, has been enabled on full production. It would be great if you could provide feedback for us. This survey may take you about 3 minutes to finish.

We are working with SharePoint team to enable file upload v2 – for external survey, we will reach out your soon for more feedback.

    Thank you for your time!

I think this will not progress anymore and seems like this idea has been abandoned.
Our company has moved to another option like iAuditor which has been great so far.

I have a suggestion: If you guys are having difficulty on how you can integrate photos or files (where is it going to be saved), why don't you look into integrating it with MS Teams. So that the Forms will not only create an excel raw database but rather a folder for certain types of files will be associated with that Forms automatically.

I am not a software engineer but, common, it should not take that long to figure this thing out.

    This is a must feature.
Example, you created an audit form for your employees or for a process within your company. After you did an audit, you want to send the result to a person. There should be an option to add an email address where you want the result of that particular audit to be sent to afterward.

This would serve as a receipt - the same concept when you are withdrawing money from an ATM. As an individual, you would want proof that you have been audited by a certain individual and which would show the performance item by item.

    Thanks for your suggestion!

SharePoint page support embed content. Currently you can copy the embed code of a form (click “Share” button, select “Embed” in “send and collect responses” section ) and past the code to SharePoint. The form will show as a web part on Sharepoint page.
We will continue thinking about improving the experience.
