Clearly show the domain of the form owner
To prevent forms from being used on personal accounts within a professional environment, as well as forms being used for social hacking / stealing personal data / phishing, there should be a clear mention of the domain of the form owner, visible to the respondent!
In excel forms it is already clear that a form belongs to domain.sharepoint.com through the URL. However, MS Forms uses forms.office.com both for personal and professional forms. There is no mention of the organisation or domain visible for the respondent.
This creates a serious risk of forms being abused to trick employees into giving away sensitive data.

Is an issue for us as well someone created a form asking for information most likely phishing but unable to tell
This is actually a big issue. As a receipent of a form I recently received from my local government, I had no idea if this form was legitimately from the government or from some random person. The URL was forms.office.com, and there was no branding on the survey. Even though the link was from a legit government email, I was concerned because the URL didn't match their usual website domain. I don't even think Forms Pro handles this.
