Grid of Choices
I would like a functionality to allow rate various choices. For example, we run training with different sessions/instructors. I would like the survey takers to rate each class by choosing a scale such as "Satisfied", "Very Satisfied", "Dissatisfied" etc.

It will be good to have more variety of forms. A check list type of form is needed where the response will be in a tabular format; with 3 options to fill: Yes , No, Remarks.
Attaching a sample for easy reference.

This is still a very important missing aspect in Forms 365. Please consider this as one of the new form types. Since There are no other ways to really make beautiful and dynamic forms and quizzes have similar flexibility would be really wonderful.

I am an avid user of Google forms and they have a function that really does need to be included in 365 Forms and that is Multiple grid type forms. I use this function to conduct evaluations and support my PhD students. We use them as checklists that show if the student is making progress. I do not use forms for quizzes I use it mostly to help me aggregate progress of my students. Also, The forms need to have the section breaks included. This is helpful to the student and to the faculty. I have attached a snapshot of how the forms with the grids are used.
