Identification
I read a few comments. I agree with many. Some good options can be entered:
1-The questionnaire shall contain the name and email of those who will be evaluated. This allows for better identification, in case some people are not part of the organization.
2-In the case of classrooms, there should also be the possibility of identification by the number of call of the evaluated pupil.2 votes -

Do not collapse choices
If you have more than 6 choices in a question it will collapse it into and expandable window. Viewing all the choices is faster (yea yeah, i know that you try to make desktop view work better on mobile, for some reason). Also translation for that collapsed field is way off. In Lithuanian it says "Select from Select the answer". It sounds worse in Lithuanian. It is just unprofessionally looking and my users turn their noses when they see such things.

Ink in Take a Test
To solve math questions it useful to have a big blank page to write the whole solution of an exercise. Make also the possibility to correct the exercise as teacher with an other colored pen.

We are creating a call monitoring form for grading our call center, not all calls are the same and therefore adding an NA option for questions that are not applicable would be beneficial so points were not taken away if NA was chosen. Example Yes = 30 points, No = 0 points, NA = 30 points so the total % is not impacted.

allow a question to serve as the identity of the respondent
Currently the forms are identified by the logged-in user - we currently use Google Forms to perform audits/assessments of certain processes, and one logged in user may audit 5 separate locations that need to be sorted/grouped, not by the person filling out the form, but by the unit that is being assessed.

Add the ability to ask for a rating on a group of items. "How would you rate us in the following areas?"
Instead of taking several questions spaces to ask over and over... create the option to ask for a rating on a group of items.
How would you rate is in the following areas?
Convenience. 1 to 5
Quality. 1 to 5
Availability. 1 to 5
Service. 1 to 5

hidden questions to validate
Make it possible to add a hidden question that you can see when you look at the answers of a filled in form. In this way you can validate the answers of the form and you can later see which results you already checked.

Allow a change to the prompt in the Text Box for a Text Question
In a Question of Type "Text", I would like to change the prompt that says "The value must be a number". The form allows one to enter text as a response, but the prompt is asking for a number. Ideally what appears in the prompt would be completely editable by the owner of the Form.

We would like an option to include an accept or decline box so that we can send out updated policies and recieve electronic feeback
We would like the ability to be able to send out updated policies and documents to our workforce and receive this information back electronically - to do this we would need an appect policy or decline policy field that would allow employees to acknowledge that they have a) received the document b) read the document and c) accept the terms of the document

Choice text boxes to be bigger so you can see what you are typing
When typing in the answers for a choice question we are presented with quite a small box which scrolls. I would like to review the text typed without having to click out of the question.

Provide an option for an even number scale
Odd number scales provide a middle option in a survey which doesn't tell us much. An even mumber is much better. Google forms can do it- it can't be hard.
