Insert the option to filter each question, so that the respondent does not see all the questions at the same time.
This makes the survey look way more professional!

that you enable a feature for the calendar with hour based schedule
i like that you have a calender option, but in my work we need an hour based schedule, so that we would get an output with 26-01-2018 at 4pm or 3:30 pm etc. maby also 24 clock.

I'm using forms for a sienctific evaluation of yammer and teams. Several features are missing to create a (good scientific) survey. First would need an extra option to add explanations between subscales. I had to write the text directly into the question space, which is not nice to read. Additionally it would be very helpful, if it would be possible to edit the text (e.g. have big letters). Finally, the "semantic differential" is often used to measure usabiliy (this is like likert scale a special art for questions). Such an option would be great!1 vote -
Change default of how many answers show up for
Make it possible to change the default add answer. The current default is choice and only 2 responses. It would be nice to change the default to 4, etc. options.

Allow for multiple 'manual' answers to one question
If we don't want it to be anonymous, we want to be able to ask them for perhaps their name, email, and company name. As it stands now - we'd have to make each of those a separate question. That looks silly.

Being able to use this for time card purposes
Being able to determine Date, Sites, Start/End times, Lunches and travel times.
Then being able to export the information as an excel file that organized by date/site/start/end/lunch/travel details so I can forward that over to payroll.

provide option to add text to the form, without it being a question.
An example would be if you wanted to say "Thank you for completing this survey!" at the end.

add "is question applicable" option, in addition to, "is required"
"is required" can be too restrictive - some questions may be required, but may not be applicable, and the consideration / "scoring" of that question, if N/A, should be discounted from totals

Make it possible to enter choices manually for a survey question
via comma separated values. Right now, I have to do it manually.

A 4-point Scale Option
The Ratings question time is on a 5 or 10 point scale. It would be great if users could answer on a 4-point scale as most companies do not like a middle option (3).
