Comment box in forms
Can we add comment box at the end of every questions in Forms. I have given option of "others" to my respondents but that is hurting my reporting purpose in excel. Since I have allowed them for multiple answering for example they have to either answer yes or no (mandatory) and for comments they can select others. but while downloading the file in excel. I have to do a lot of work to separate the responses since comments has been treated as a choice.

Have the ability to put instructions before the question. For example Please rank the following from 1-5
We have the ability o add scale to the questions. They can go from 1-5. Would be good to be able to add instructions Above the start of the group of questions. This helps to prevent the question of What do we do with these questions. (i know it should be self explanatory but I am still getting questions.)

Repeating table support in Forms
It would be great the support for using a repeating table (InfoPath) as formfields.
So the user can add more rows to a form31 votes -
enable the ablitity to rearrange listed answers within the multiple choice form builder section.
I suggest:
1) the ability to edit form responses as needed and
2) enabling the ability to rearrange listed answers within the multiple choice form builder section. This way the user can add optional multiple choice answers without having to delete previously recorded answers, rearranging the order as needed.

Create a sliding scale and numerical scale. With options to design the exact length or number options on a horizontal scale.
A horizontal or vertical scale that can be ranked to set numbers identified by the designer of the question. Or a sliding scale option where the individual can slide a cursor along a horizontal line to indicate their level of agreement along a scale that has an anchor on each end.

Checkbox grid
PLEASE add Checkbox grid like Google forms. You really need to add this functionality. Microsoft should be at least as good as Google forms if not better and it just isn't. My company recently started using Teams and O365 business and we really need additional question types for forms.

Dynamic Content for Forms Questions/Responses
One of the Question types should populate/verify dynamic from an excel spreadsheet
Dynamic Content for Questions: Pull questions from a linked sheet that will allow forms to modify/update that row
Dynamic Content for answers: Say you have hundreds of devices, and you want your team to answer a question about a specific device. The form should match that device with dynamic content of an excel sheet like flow does. "List devices in column XXX"

String Validation
Add the possibility to add basic validation upon input fields according to its type.
For string would be nice to match lenght and at least the occurence of spaces or other special characters.
For dates the match would be before/after some day.
Boolean and currencies types are missing.28 votes -

GPS location
GPS Location & Coordinates - Integration with Bing Maps allowing the Forms author to add a location, or the respondent to add GPS coordinates and a map screenshot. For example, one question could be "Where are you?" and the answer will be GPS based and pin on a Bing Map. For the results summary, the Forms owner will see the location of all respondents. Or, on a quiz, the question could be to name the country shown on the map. For a respondent, they may want to include a GPS coordinate to further describe an answer.

Allow user to write comments for a rating question.
When a user is asked to rate something from 1 to 10, allow a text field to "explain the rating" or "add comments" to the rating

Allow to make a text box appear below a selected checkbox
There is a feature to allow people to select more than one item, so it transform the radio button into check boxes to allow multiple selections.
But why not adding this feature : being able to add a TEXT box that would appear below any selected item and it would let the user explain WHY he chooses this option, or check box. Labeled whit something like : Explain why you choose this answer.

Offline data collection
Develop offline data collection - so that the form can be answered by any device without an internet connexion, that then syncs when an internet connection is available.

Add a question from another form
When adding a question, give an option to add a question from an existing form. This effectively means all questions ever used can be accessed, almost like a question bank. This would save tonnes of time.
So you click Add Question, click Add Question from Form..., select the Form you want to get a question from, then the questions on that form are displayed with an add button next to them. You can add multiple questions from that form and hit finish when done. Something like that!27 votes -
simple yes/no buttons
I know you can create yes/no questions with choice/radio buttons but it would be a good addition to have a yes/no button alternative to yes/no questions

multi part questions
Multiple part questions
Ability to create a question with different related answers in one question instead of multiple questions.
EG.
Training Required?Topic, Participiants, Date, Duration and Mandatory
Have that as one question instead of 5 different questions
Results can be treated as separate columns in excel e.g. Question 5.1 5.2, 5.3,5.4,5.5 but displayed as one question when completing the input on the form.
